ORTHOPAEDIC SURGEON
Dr Spike (P.J.) Erasmus
Dr Spike Erasmus is an orthopaedic knee surgeon
Erasmus is a world authority on inverse kinematic alignment in total knee replacement and patella instability. He now limits his practice to knee replacement and patellofemoral surgery.
In knee arthroplasty, he pioneered the use of inverse kinematic alignment (originally called “bone balancing”) and has refined this technique over the past 15 years.
In patellofemoral surgery, Dr. Erasmus developed an anatomic MPFL reconstruction technique, first presented at ISAKOS in 1997. His method has become the most widely used surgical approach for recurrent patella dislocation worldwide.
Three of his MPFL publications are ranked among the top 100 most-cited articles on the subject, as highlighted in a 2024 bibliometric analysis in The Orthopaedic Journal of Sports Medicine.
In 2006, he introduced the use of intravenous dexamethasone in knee replacement surgery, a technique that is now used globally.

Dr Erasmus has published extensively in peer-reviewed journals and has lectured at major international orthopaedic conferences. He is widely respected for his innovation, surgical precision, and dedication to patient-centred care.
